• Advocacy – act as an advocate for the client in health care issues, hospitalizations, discharge planning, facility care and placement, family concerns, legal issues, and all aspects related to the client’s health and well-being.
  • Ensure that care and support being provided to the client is in the best interest of the client.
  • Disease process, chronic disease management and treatment plan. Provide education on signs and symptoms that can trigger a medical emergency.
  • Education of family and caregivers on home care needs and developing a plan of care to meet needs and expectations.
  • Providing resources, options and solutions to clients and families
  • Assistance accessing the social services, resources and health care team members to maintain their quality of life.
  • Education on long term needs and planning for the future.
  • Benefits – what is available and how to access these benefits. Medicare, Social Security, Medicaid, Long Term Care Insurance and Veterans Benefits.
  • Coordination of care with multiple physicians and agencies.
